Who this is for
Men with symptoms that can come from low testosterone:
- Low energy or fatigue that sleep doesn't fix
- Lower sex drive or fewer morning erections
- Harder to build or keep muscle
- Low mood, irritability, or brain fog
These symptoms overlap with poor sleep, thyroid problems, depression, and medication side effects. That's why we test before we treat.
How it works
- Video intake. Your symptoms, health history, medications, and goals, including whether fertility matters to you now or later.
- Lab testing. Morning labs, usually on two separate days: total and free testosterone, plus LH, FSH, estradiol, a blood count, and PSA when appropriate. Insured patients can run labs through their own insurance.
- A plan that fits. If the results and symptoms line up, we choose a treatment together.
- Ongoing monitoring. Follow-up labs, home blood pressure readings, and regular check-ins by video or message.
Treatment options
Clomiphene
A daily pill that signals your body to make more of its own testosterone. Because your natural system stays active, sperm production usually keeps running. A good fit for men who want to preserve fertility or avoid injections, when the testes are working but the signal from the brain is low. Clomiphene is an FDA-approved medication available as a generic; using it for low testosterone in men is off-label. How clomiphene works →
Testosterone replacement therapy (TRT)
Testosterone as a weekly injection or a daily cream. TRT is the standard treatment and works well, but it can lower sperm production, so it needs closer monitoring of blood counts, estradiol, and PSA. We walk you through your first injection live on video.
hCG alongside TRT
For men on TRT who want to help preserve testicular function and fertility, FDA-approved hCG can be added to the plan when appropriate.
When we'll refer you elsewhere
- Very low testosterone with signs of a pituitary problem
- Active prostate or breast cancer, or untreated severe sleep apnea
- Anything that needs an in-person exam to evaluate safely
Which plan
Hormone covers clomiphene, with the labs and check-ins it needs.
Hormone+ covers testosterone, injectable or cream, with closer monitoring. Medication is included in both. Add ED and hair, or step up to Total for ED, hair, skin, and hormones plus sick-day visits.
Testosterone is a controlled medication and requires an evaluation and regular follow-up.
Pricing goes to the waitlist first. HSA/FSA cards can be used for eligible medical services. Founding membership opens November 1.
Common questions
Do I need labs before starting testosterone?
Yes. Low testosterone is diagnosed with morning blood tests, usually repeated on a second day, alongside your symptoms. We also check markers like LH, FSH, and estradiol to choose the right treatment.
Will TRT affect my fertility?
It can. Testosterone from outside the body often lowers sperm production. If fertility matters to you, clomiphene or adding hCG to TRT are options we can discuss.
Can I get testosterone through telehealth in New York?
Yes, for patients located in New York State, after a video evaluation and appropriate lab testing. Testosterone is a controlled medication, so it requires regular follow-up.
Can I use my insurance for labs?
Yes. Insured patients can have labs billed to their own insurance. The membership covers the clinical care and medication.
