Flintlock Health provides care by telehealth only. This page explains what that means so you can decide whether it's right for you. You'll review and accept this consent in your patient account before your first visit.
Who provides your care
Care is provided by Andrew Charles Storer, Nurse Practitioner in Acute Care, PLLC, doing business as Flintlock Health. Your clinician is a nurse practitioner licensed in New York. Flintlock Health LLC provides non-clinical services such as the website, scheduling, and billing, and does not make medical decisions.
How visits work
- Every new patient starts with a video visit with your clinician. Follow-ups may be by video or secure message.
- You must be physically located in New York State at the time of every visit. We'll confirm your location each time.
- You must be 18 or older and provide accurate, complete health information.
Prescriptions
Prescriptions are written only after your clinician evaluates you and decides treatment is appropriate. A prescription is never guaranteed. Prescriptions are sent electronically to a licensed partner pharmacy, which ships to you in New York.
Testosterone and modafinil are controlled substances. They require a video visit, lab testing where indicated, a check of the New York prescription monitoring program, and regular follow-up visits. Missing required follow-up or labs may mean we can't continue the prescription.
Benefits and limits
Telehealth means no waiting room and less travel. It also has limits: your clinician can't do a hands-on physical exam, and some conditions need in-person care. If your clinician decides you need an in-person exam, lab work, or specialist care, we'll tell you and help you find it.
Risks
- Video or connection problems may interrupt or delay a visit.
- Information you share could be incomplete, which could affect your care.
- Although we use secure, HIPAA-compliant systems, no electronic system is perfectly secure.
Emergencies
Flintlock is not for emergencies. If you have chest pain, an erection lasting more than 4 hours, trouble breathing, signs of a stroke, or any other emergency, call 911 or go to the nearest emergency room.
Your privacy and records
Your health information is protected under HIPAA and New York law, as described in our Notice of Privacy Practices. Visits are not recorded without your permission. Your records are kept in our electronic health record and you can request a copy any time.
Your choices
You can decline telehealth or stop care at any time without penalty, subject to our Refund & Cancellation Policy. You may ask questions about this consent before you agree.
